40例肾动脉狭窄血管内介入治疗降压疗效分析_肾动脉狭窄介入治疗

［摘 要］ 目的 探讨血管内介入治疗肾动脉狭窄患者 的降压疗效。方法 回顾分析40例肾动脉狭窄（狭窄>70%) 患者行经皮肾动脉球囊血管扩张成形术(percutaneous transluminal balloon angioplasty , PTBA）和／或经皮肾动脉支架植入术(percutaneous transluminal renal artery stenti ng, PTRAS）的疗效，观察术前术后血压。结果 总共治疗62支肾动脉，其中8支肾动脉行PTBA,54支肾动脉行PTBA+P TRAS,均成功。患者收缩压由术前（158.93±26.20）mmHg降至术后第一天的（138.45±2 2 .46）mmHg（P30%或出现严重并发症需中转手术。降压 疗效：［4］（1)治愈：在不服用降压药物的情况下血压 肾动脉狭窄造成的严重临床事件（包括不稳定型心绞痛、与心功能不匹配的肺水肿、难治性高血压和缺血性肾病）日益受到广泛重视。目前肾动脉狭窄介入治疗技术渐趋成熟，临床应 用日益增加，但是其临床疗效存在很多争议［6］，临床疗效各研究不尽相同。ASTRA L和STAR研究显示肾动脉狭窄血管内介入治疗与药物治疗在控制血压及改善肾功能疗效上没有差异［7，8 ］。但SNRASCG研究发现，双侧肾动脉狭窄患者行PTBA术较药物治疗组虽然肾功能没有明 显改善，但收缩压有轻度改善［9］。Burket等报道［2］，肾动脉介入治疗显 著降低患者血压，减少患者服用降压药物数量。
本文提示，肾动脉狭窄血管内介入治疗，微创、安全，可以较好的控制血压，减少服药种数；但其临床获益尚需大规模循证医学研究验证。
