　　[摘要] 随着围产医学的发展，产科质量的不断提高，人们（特别是剖宫产产妇）对减轻和消除疼痛的问题越来越关心。我院自2011年7月至2011年11月对31例剖宫产妇术后开展全麻镇痛以来，效果满意。现报道如下。目的：观察在全身麻醉用于剖宫产对产妇及胎儿的影响。方法：选择我院急诊剖宫产手术31例，采用气管插管静脉复合麻醉，药物以丙泊酚、琥珀胆碱、氯胺酮、芬太尼为主，观察该方法对产妇及胎儿的影响。结果：诱导平稳，麻醉满意，无呕吐误吸和返流发生，无术中知晓，对胎儿Apgar评分无明显影响，对子宫收缩无抑制。结论：全身麻醉为剖宫产可以选用的安全方法之一。

　　[Abstract] With the development of perinatal medicine, obstetrics and continuously improve the quality of people （particularly cesarean section） to reduce and eliminate pain is increasingly concerned about. Our hospital from July 2011 to November 2011 after 31 cases of cesarean section in women analgesic anesthesia carried out since the results were satisfactory. Are reported below. Objective To observe the general anesthesia for cesarean section on maternal and fetal effects. Methods hospital emergency cesarean section in 31 cases, intravenous anesthesia with endotracheal intubation, drug propofol, succinylcholine, ketamine, fentanyl-based, to observe the method of maternal and fetal effects. The results induced stable, satisfactory anesthesia, no vomiting, aspiration and reflux occurred, no intraoperative awareness, the fetus A pgar score had no significant effect on uterine contractions without inhibition. Conclusion general anesthesia for the cesarean section can be used to secure one of the methods.

　　全麻可消除产妇的紧张恐惧的心理，麻醉诱导迅速，低血压发生率低，能保持良好的通气，适用于精神高度紧张的产妇或合并精神病、腰椎疾病的，有区域麻醉禁忌症的，区域麻醉失败的或者在行剖宫产术时同时计划进行其他手术的。但是由于产妇特殊情况，采用全麻时容易呕吐或反流误吸，甚至死亡。麻醉用药不当有造成新生儿呼吸循环抑制的危险，难以保证母儿的安全。因此，全麻在一般只有区域麻醉或局部浸润麻醉禁忌时采用。本研究通过31例剖宫产来探讨全麻插管的安全性和麻醉的满意度，以及对胎儿的影响。
　　1 资料与方法

　　选取了31例需紧急行剖宫产的单胎孕产妇，年龄25--35岁，11例先兆子痫（患者烦躁不配合椎管内麻醉），10例患者肥胖，导致穿刺失败，10例血小板减少症（PLT
